Radekal Review

Radekal is an electronic health record (EHR/EMR) solution created to simplify the process of recording patient exams and automatically generate history and physical exam (H&P) notes. The system is designed to reduce the time it takes clinicians to perform and document patient exams by simplifying the data entry process. Swipes can be used to mark multiple fields in the system review (ROS) or physical exam (PE) as positive or negative, so users must manually document any abnormal signs. Anatomical drill-down diagrams allow users to show the location of any radiating symptoms or pain. Procedures, diagnoses and study prescriptions can be added at the end of the exam and users can set up a “hot list” in the Rx tab for quick access to commonly prescribed medications with pre-filled information on strength, dosage, frequency, interval and duration of treatment.

RADEKAL – FEATURES

• Users can complete the physical exam documentation with a single tap for a pertinent positive, double tap for a pertinent negative, upwards swipe to mark all parts of the physical exam positive, downwards swipe to mark all parts of the review of systems (ROS) negative, and interactive anatomical diagrams to identify the affected areas.
• Annotations can be added during the exam or on the final note, and users can dictate annotations if they are using a tablet device
• Complete history and physical examination (H&P) notes are generated automatically, including all information on patient details, chief complaints, history of present illness, review of systems (ROS), Past, Medical, Family, and Social History (PMFSHx), active medications, allergies, physical exam and vital signs, assessment (diagnosis), lab orders, prescriptions, evaluation and management (E&M) codes, and counseling time
• History and physical examination notes and prescriptions can be signed electronically, and users are automatically alerted before signing if the Medical Decision Making (MDM) section has not been completed
• A ‘hot list’ can be set up to give users easy access to commonly prescribed medications with prefilled information on the strength, dosage, frequency and interval, and duration of treatment.
